Target Audience

Physicians of all specialties and advanced practice providers caring for long term care patients, especially those who are not based in the nursing home but oversee care from their office practice. Other interested healthcare professionals are also welcome to participate.

Description

Nursing home patients today are older, sicker, and more frail than ever before. Multiple chronic conditions, reduced mobility and cognition put them at risk for polypharmacy, falls, malnutrition, and pressure ulcers. Primary care physicians not based in the nursing home have the disadvantage of practicing remote medicine. If end-of-life wishes have not been discussed, patients are at increased risk for aggressive and futile care in the final weeks and months of life. These situations create pain and anguish for the patient and family, and increased liability risk for the physician. This on-line activity seeks to arm the physician with specific strategies and resources that will assist in making the care of nursing home patients a reasonable and manageable risk.

Learning Outcomes/Objectives

Upon completion, participants should be able to:

  1. State 3 things you can educate patient/family prior to nursing home admission that will reduce risk.
  2. Describe 3 strategies that can reduce communication errors in the care of nursing home patients.
  3. Differentiate palliative care and hospice care and state how these services may benefit nursing home patients.

ABIM

Successful completion of this CME activity enables the participant to earn up to 1.00 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC points.
